Information on required documents and forms for all members
Status of Forms:
The Form Tracker spreadsheet here 2010-2011 Form Tracker.xls indicates which forms are required and have been received for the upcoming pack year. An ‘X’ in a cell indicates that the form has been received, and a blacked out cells indicate the form is not required. A significant percentage of the pack still needs to submit updated medical forms. Only sections "A" and "C" need to be filled out.
Please note that medical forms (Sections A & C) are required for ALL Cubs and ALL Dads and must be on file to attend Blake Island.
The medical form is available here BSAMedicalForm.pdf can either be handed in via paper copy to Keith Olson at the Wednesday pack meeting, or submitted via e-mail to Keith membership@pack144.org.
Form Requirements:
Cub BSA Applications:
Cub applications are required for all new cubs including legacies.
Cubs returning from last year do not need to resubmit.
Adult BSA Applications:
Adult applications are only required for Den Dads and Akelas. They only need to be filled out once.
